The following information will be necessary:
MOVING IN
- Date your service will begin
- Name
- Address
- Phone Number
- Mailing address if different from residence
- Do you own or rent? if Renting - whom is your landlord?
- Driver License, Social Security Number, or Passport Number
MOVE OUT
- Date you will be ending service
- Name
- Current Address
- New Phone Number
- New Address for your final bill
